COVID-19 Virtual Follow-up Clinic
You have been assessed for the COVID-19 virus and provided with a pulse oximeter. To help you to monitor yourself at home, there is an app - called Connected Care PHR. The app has a Care@Home section in which you can record your symptoms and readings whilst on the virtual ward. This is part of a new Personal Health Record (PHR) app designed to help you better manage your own health and care
The Connected Care PHR app is available on smartphones (both IOS & android) and can be downloaded using the links below.
As part of the Virtual Follow-up Clinic, you can use the Care@Home section of the app as an alternative to a paper diary to monitor your progress until you are well again. (Please note that if you do not see the Care@Home section when you log in, you need to contact the virtual clinic team to check that you have been enrolled.)
You will be contacted by telephone daily, at a pre-arranged time, until your symptoms have improved and you are well enough to be discharged from the virtual ward.

How to access the PHR
The app can be downloaded here:
Open the Connected Care app and click NHSLogin. If you already have an NHSLogin, use your email and password. If you don't have an NHSLogin, you can enter your email address and begin the process to create your NHSLogin from within the app.

The COVID-19 virtual follow-up clinic service is currently available in Slough, Windsor and Maidenhead.
