Healthier You: NHS Diabetes Prevention Programme (NDPP)
You have been identified by your practice as being at risk of developing type 2 diabetes due to a blood test within the last 24 months. You are therefore eligible to be referred onto the free NHS Diabetes Prevention Programme (NDPP).
This programme is an evidence-based behaviour change programme that will support you to make small lifestyle changes to make healthier food choices, increase physical activity and lose weight. Although the programme is normally delivered in groups at different times and locations, right now it’s offered over telephone or group video conference, or online through apps and websites
Referral onto the NDPP will include:
- 9 months’ free support and guidance from trained coaches
- Access to online support through apps and websites
- Weekly remote workshops
- Online community groups to network and connect with individual’s like yourself
- 1:1s support and motivation with access to a coach via 24/7 chat
- Workbook with all session content included to refer back to

Preventing type 2 diabetes
Diabetes can increase your likelihood of developing heart disease or having a stroke. It is responsible for most cases of kidney failure and lower limb amputation, other than accidents, and is the leading cause of vision loss and blindness in people of working age. So it is important for you to engage with this programme, as Type 2 Diabetes can be prevented through small lifestyle changes.
