Weight management
There are many weight loss and weight management programmes that are available to help people to lose the pounds and to keep them off.
Everyone is different, so depending on personal circumstances a person may get along fine with a few tips or maybe some signposting to some activities and organisations.
If something a bit more structured or intensive is needed to succeed, there are options there too.

Lifestyle Interventions

A nine-month supported lifestyle intervention for people with Non-Diabetic Hyperglycaemia (NDH). Referral via HCP (referral form on DXS GP referral system) or self-referral using Know Your Risk tool. |
Digital support (via smartphone or other internet-accessible device) for adults living with obesity AND either diabetes, or hypertension (or both), to help manage weight and improve health. |
A one-year programme for people who have been diagnosed with type 2 diabetes in the past 6 years and have a Body Mass Index of 27 or above (or 25 or above for people of Black, Asian or minority ethic origin) which includes 12 weeks total diet replacement. ShapeUp4Life (Hampshire only) | A free 12-week weight management programme for Body Mass Index (BMI) of 30 or above (or 28 or above for individuals with specified long-term conditions, or Black or minority ethnic groups). Available via referral by healthcare professional or by self-referral. |
One You Surrey (Surrey only) | Offers a range of programmes including 12-week weight management programme, digital weight loss programme Gloji, Slimming World or MAN v FAT. For patients with Body Mass Index (BMI) of 30 or above (or 27.5 or above for Black or minority ethnic groups). Available via HCP referral (referral form on DXS) or self-referral. |
The Health Coach (Aldershot only) | Behaviour change programme for people in Aldershot with pre-diabetes and a Body Mass Index (BMI) over 28. Available through HCP referral. |
MAN v FAT (Hampshire only) | For men with a Body Mass Index (BMI) of 27.5 or above who want to lose weight, improve fitness and play football with like-minded men. Available through self-referral via the website. |

Tier 3: Specialist service
For residents in Bracknell Forest, the Royal Borough of Windsor and Maidenhead and Slough, there is a speciality obesity service weight loss programme provided via the Royal Berkshire Hospital NHS Foundation Trust. The service is only for patients who meet the criteria for Tier 4 and intend to progress to bariatric surgery. Referral via ERS.
For residents of North East Hampshire and Farnham, a focused programme of weight management is provided via Spire Southampton Hospital which, in addition to diet, includes exercise and psychological support. Referral via ERS.
Tier 4: Bariatric Surgery
For patients in Bracknell Forest, Royal Borough of Windsor and Maidenhead and Slough, surgery is provided by the Royal Berkshire Hospital NHS Foundation Trust. Referral via ERS.
For patients in North East Hampshire and Farnham, surgery is provided by Spire Southampton Hospital. Referral via ERS.
For patients in Surrey Heath, surgery is provided by St Peter's Hospital. Referral by ERS.
